How to File Your Wellness, Health Screening, or Benefit Claim
Filing your annual wellness and health screening claims are important to your health. Each claim pays independent of the other, is available for family members, and payment is tax free.
The plans available are:
- Group Accident Insurance, with a wellness benefit of up to $60
- Group Critical Illness Insurance, with a wellness benefit of $100 (does not pay for dependent children)
- Group Hospital Indemnity, with a wellness benefit of $50
- Cancer Insurance, with a wellness benefit of $100
To file a claim with Aflac (annual wellness, health screening for accident, critical illness, and hospital indemnity), you can access it directly through this website: www.aflacgroupinsurance.com/customer-service/file-a-claim.aspx.
